What’s the lifespan of the lights? Does the portrait frame only light up or are the tattoos illuminated as well?

They are LED Lights so they should last for the average LED lifespan (50K hours).
The portrait should light up as well as giving the canvas itself a colored hue.

Is the portrait back-lit with blue LEDs that light the outside as well as the tattoo/robotics?

There will be a light hue for the canvas. The lights will mainly light the outside portrait of it.

Do the lights actually shine through the portrait or are they only on the outside of the frame?

They light through the sides and give off a hue for the actual canvas.
